These forms are available for active Law Enforcement/Corrections Officers only through approval of the Lodge. FOP Active Members must already be a member of FOP Legal Defense to qualify for the life insurance benefit which is provided at no additional cost. If you do not have Legal Defense coverage and would like to this to your membership, complete the Legal Defense Application and send it here.
